Information provided on the statement will conveniently show the date and dollar amount of each transaction, the signature of the employee making the purchase, and the location where the purchase was made. Plus, these reports allow business owners to check vehicle and driver efficiency, making the management job easier.
Then, they pay once a month from one easy-to-read statement. That's it! Wait 20 days to pay!! Payments are due by the 20th of the month for a prior month's charges.

- Fill-in the requested information on the form.
- When done, print out application.
- Make sure to sign & date the application.
